摘要

A68-year-oldwomanwithfever,chills,asthenia,nightsweats,arthralgia,weightloss,andnecrotizingskinulcersonthebodyandlowerextremitieswasadmittedtoourdepartment.Shehadbeenfollowedforrheumatoidarthritis(RA)forover20years,andherrheumatoidfactor(RF)levelwasveryhighatadmission.Amonthago,atemporarypacemakerwasappliedtothepatientduetodrug-inducedbradyarrhytmia,butthepacemaker’sleadfracturedandremainedinsidetherightventriclewhileitwasremoved.Revealingthevegetationsonthefracturedleadandtricuspidvalvebyechocardiographyandmethicillin-resistantcoagulase-negativeStaphylococcusaureusinseparatebloodcultures,thetwomajorcriteriaforthediagnosisofinfectiveendocarditis(IE)wasconfirmedto
